Add Enrollment Management>School Forms>Block>Household>Maiden Name as a choice.

Enrollment Admin>School Forms>Update Spouse Information> need to add the field "Maiden Name" as an option/choose in the Block>Household Information. (This is Form type>Profile Update.)
