Allow Form Fields in Nurse's Medical forms

I am submitting this for our School Nurse.

We were trying to create a Covid-19 Questionnaire for students to complete. We would love for it to go directly into their medical record via "Medical forms" but there is no form field option to select to create the form. I feel as though this functionality will have further reaching capabilities beyond COVID, as there may be other Health Screening Questionnaires that could be customized that the School Nurse's would want to go directly into the Medical Record. For now the work around is to created the form in Core, download the form, then add to the medical record. This is a time consuming process for an already busy office.

    Yes please! It's the only form type that doesn't have the ability to add the "Form Field" block. We need to collect just a little more information on this form!
